1. Acceptance of Terms

By accessing or using the Celimot website or booking a consultation, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.

2. Description of Services

Celimot provides telehealth mental health consultations via secure video, delivered by Taiwo Okubanjo, a licensed Psychiatric Mental Health Nurse Practitioner (PMHNP), licensed by the Texas Board of Nursing. Services may include mental health assessments, follow-up consultations, medication management, and talk therapy sessions.

3. Eligibility

Our services are available to individuals who:

• Are 18 years of age or older
• Are physically located in a state where our provider is licensed to practice at the time of the consultation
• Can provide accurate, complete information during intake

4. Scope of Practice

As a Psychiatric Mental Health Nurse Practitioner, Taiwo Okubanjo provides assessment, diagnosis, treatment planning, medication management, and supportive counseling within her licensed scope of practice. Cases requiring services outside this scope will be referred to an appropriate specialist.

5. Booking, Fees & Cancellations

• Consultation fees are displayed at the time of booking and are due at the time of service unless otherwise agreed.

6. Prescriptions

Prescriptions are issued solely at the clinical judgment of the treating provider, based on the information available during your consultation. Not all conditions or requests will result in a prescription. Controlled substance prescribing via telehealth is subject to additional federal and state regulations.

7. Patient Responsibilities

You agree to:

• Provide accurate and complete information during intake and consultations
• Use a private, secure location and stable internet connection for video visits
• Follow the treatment plan discussed, or communicate promptly if you cannot
• Notify us of any adverse reactions to prescribed medication

8. Technology Requirements & Limitations

Video consultations require a device with a camera, microphone, and internet connection. Technical issues (dropped connections, poor audio/video quality) may occasionally disrupt a session; we will work with you to reschedule if this occurs.

9. Limitation of Liability

To the fullest extent permitted by law, Celimot and its providers are not liable for indirect, incidental, or consequential damages arising from use of our services, except where such limitation is prohibited by law or in cases of gross negligence or willful misconduct.

10. Intellectual Property

All content on the Celimot website (text, graphics, logos) is the property of Celimot and may not be reproduced without permission.

11. Termination of Services

We reserve the right to decline or discontinue services to any individual, including for non-payment, abusive conduct, or when continued telehealth care is not clinically appropriate — in which case we will provide a referral where possible.

12. Governing Law

These Terms are governed by the laws of the State of Texas, without regard to conflict of law principles.

13. Changes to These Terms

We may update these Terms from time to time. Continued use of our services after changes are posted constitutes acceptance of the updated Terms.
